After upgrading to the newest drivers, when I try to apply this patch it will not recognize my card ( 770 ) - and I have to uninstall the driver, and re-install in order to get both monitors back, or even to get back to the correct resolution.

The patcher has been confirmed to work with 334.89. I'm not seeing any problems. It finds all the limits, and someone with an SLI setup confirmed that it works with 334.89.

The only thing I can think of that would cause something like that is if the driver is failing to load because of a code signing problem, and if that were the case, the patcher should have given you an error message, but you didn't mention getting any errors, so I don't know why you're having this problem.

You shouldn't have to uninstall the driver to undo the patch. The patcher has a restore feature. If you run it again, it will give you the option to restore the unpatched driver.

Running latest Nvidia drivers. Single card setup. Latest windows updates as well(Probably my problem). I get this when I try to run it. It worked before. It also deletes the file as you can see only the full version is there when the other was there before.
