The 74 hz works, but the sides go offscreen on every resolution. What should I do?
Then the monitor doesn't handle the resolution correctly. The only thing you can do is try other timing parameters, but if the standard/native/reduced presets don't work, then it probably can't do it.

it was working fine yesterday, I restarted from the .exe and it started doing this..

I also was using 1728x1080 74 hz as a custom res in-game. But now it only shows up when I use 60, does this mean it got kinda damaged so I can't use 75 anymore?

No, that shouldn't be the case. You said 1920x1080 @ 74 Hz. Are you using VGA?

Yes, it's VGA. My monitor is ASUS VS228DE

The problem with VGA is the monitor has to guess where the pixels are, and VGA monitors tend to automatically adjust after every resolution change, so changing to an unsupported resolution might mess up the alignment. Pressing the auto adjust button should fix the alignment.

I need more details. What video card and monitor? Are you using DisplayPort? HDMI? What resolutions are you adding?

GTX 970 - Benq Zowie XL2720 - DVI-D dual - Resolutions like 1440x1080 or 1920x1080 all with 200hz sometimes 199hz

I been using this for years

This is probably a patcher issue. There must be a new limit. Can you check if you can add a resolution under 330 MHz pixel clock and between 330-340 MHz? I'm thinking there's another limit that prevents resolutions beyond 340 MHz from showing up, but I don't have a way to test it myself.

Wow that's true it's a patch issue I should have tested first

Can you verify which pixel clocks are working?
